Below is a sample of an HIPAA Authorization and Consent of Information form that we filed on behalf of our client authorizing the release of all their medical records-mental health records, billing statements, radiological films, pathology material, photographs, videos, and other information-to our firm. This document also allows our client’s medical provider to verbally disclose information about our client’s diagnosis, treatment, and prognosis. Baltimore Metropolitan Council Evaluates Whether To Add Rest Stops For Trucks Between The Beltway And Pennsylvania Line

Heavy freight traffic on Baltimore highways is causing the Baltimore Metropolitan Council to review whether there needs to be more rest facilities available to truckers. There are currently no designated rest stops between the Beltway and the Pennsylvania line, and the council wants to determine whether more truck parking areas need to be created. The council’s Truck Parking Partnership Study is looking into this issue and will make a recommendation.

According to federal regulations, truck drivers:

-Must park and stop if they’ve been driving for 11 hours straight.

Traffic-Slowing Devices To Be Permanently Installed In Salisbury’s Newtown Historic District

In Salisbury’s Newtown Historic District, pedestrians and drivers can rest a little easier when navigating through their streets. Devices designed to slow traffic are going to be permanently added to the neighborhood.

A landscaped median will be installed at the entrance of Route 13 and at Division and Mill Streets, while four-way

stop signs will replace a blinking light at Division and Isabella Streets. Curbs located between Mill Street and Route 13 on Isabella Street will be bumped out and landscaped to cause traffic in the area to slow down.

Maryland Car Accident Fatalities Reduced By Laws Placing Limits On Licensed Minors, According To AAA Report

The AAA Foundation for Traffic Safety has released a report saying that restrictions imposed on teen drivers by the state of Maryland may be working. According to the report, measures known as “graduated driver licensing” can reduce car accident fatalities involving young drivers by 20%.

Limits for licensed minor drivers in the state of Maryland include:

*Not being able to carry passengers younger than 18 years of age when driving unaccompanied by a qualified supervising driver during the first 5 months of being licensed.

Maryland Natural Resources Police Investigate Recent Boating Accidents

The Maryland Natural Resources Police have had to turn their attention to local waters where a number of boating accidents have recently taken place.

On June 18, In St. Clement Bay near St. Clement Shores, a 10-year-old boy was injured in the head when an 18-foot boat hit a pound net. The child was flown to Washington DC and treated at the Children’s Hospital National Medical Center.

Just one day earlier, the NRP recovered the bodies of two men from the Potomac River. They had died in a boating accident on June 16. Their boat had been found without anyone on board just south of the Woodrow Wilson Memorial Bridge. A third person who survived the accident was taken to a hospital for treatment. The NRP is investigating the cause of the accident.

Family Files $5 Million Dollar Wrongful Death Suit Against Delaware-Based Air Ag Inc. For Negligent Use Of Pesticide Spray

A Maryland family recently filed a $5 million dollar wrongful death suit against Air Ag Inc. and its owner Robert J. Collins for the death of Edward Louis Brittingham in June 2003. The lawsuit claims that Brittingham died after he was exposed to Warrior, a highly toxic pesticide, when the Delaware-based aerial pesticide application company sprayed it onto a wheat field near a residential area in the city of Berlin.

The suit, filed on June 8 in Worcester County on behalf of Brittingham’s widow and five children, alleges that Air Ag Inc. and Collins were negligent because they “knew or should have known that the pesticide being applied was highly toxic and would cause serious injury including death if improperly or negligently applied.”

Causes for filing wrongful death suits in Maryland include:
